The utility shall provide data in the above format at the year-end for the preceding three years and current year. In addition the licensee shall provide data in the above format as on 31/3/65 & 30/9/91 in separate tables.

Information shall be provided by voltage class (33 kV.11kV.0.4/0.22 kV0 as far as practicable in separate tables.  Repeat the same format to provide voltage class-wise information.

Attach supporting documents for each of the entries made.

Assumptions made, if any, should be clearly indicated at appropriate places.
